CRITÈRES D'ÉLIGIBILITÉ	Les plus pertinents Âge 18-80 ans Première ligne de traitement pour histologie CD20+ LNH diffus à grandes cellules, NOS (germinal ou activé) Lymphome B riche en cellules T/histiocytes LNH diffus à grandes cellules, NOS EBV positif LNH B à grandes cellules ALK positif LNH DGCB Kaposi associated herpesvirus/HHV8 LNH DGCB/haut grade avec réarrangement MYC et BCL2 LNH B de haut grade, NOS Score IPI 2-5 ? ECOG 0, 1, 2 Espérance vie de plus de 6 mois Maladie mesurage FEVG ≥ 50% VIH nég ou si VIH + : CD4 plus 200, CV indétectable et sous antirétroviral Contraception
CRITÈRES D'EXCLUSION	Les plus pertinents Greffe d’organe solide Contrindication à glofitamab ou Pola-R-CHP Neuropathie périphérique grade 2 et plus LNH transformé, LNH folliculaire grade 3B, LNH primaire médiastinal, LNH diffus à grandes cellules primaire cutané Corticostéroïde > 50 mg prednisone. Exceptions permises (voir protocole) Cancer solide antérieur (exceptions permises) Maladie cardiaque, hépatique Atteinte SNC TB latente Grossesse
